Alternatively you may contact the Surgery and ask to speak to the Operations/ Complaints Manager Jessica Hamer, as your feedback is very important and valuable to us.  

Please note : only use this form for comments about the practice and suggestions as to how we can improve our service to you.

Medical matters and official complaints cannot be dealt with via this form. If you have a query regarding a medical matter please telephone reception to make an appointment to see the appropriate person.

We make every effort to give the best service possible to everyone who attends our practice.

However we are aware that things can go wrong resulting in a patient feeling that they have a genuine cause for complaint. If this is so, we would like to reassure you that we will make every effort for the matter to be settled as quickly and as amicably as possible.

To pursue a complaint in the first instance, please contact our complaints manager Jessica Hamer who will discuss and investigate your concerns.

Alternatively you may complete the attached form, and send for the attention of Jessica Hamer who will deal with your concerns appropriately.

Please note that if you are raising a complaint on behalf someone else we will require them to fill in a third party consent form. Please contact the surgery.
